Plan G = almost-all-inclusive. You pay the small Part B deductible ($257 in 2025) and nothing else.
Plan N = lower monthly premium, but you handle:
Up to $20 copay for doctor visits.
Up to $50 copay for ER visits that don’t lead to admission.
Any Part B excess charges (extra fees some doctors can bill above Medicare’s rate).
Both plans pay 100 % of hospital coinsurance, skilled-nursing coinsurance, the Part A deductible, blood, hospice coinsurance, and 80 % of foreign-travel emergencies. Coverage is federally standardized, so a Plan G (or N) from one carrier is medically the same as any other.
No visit copays. Zero at the doctor, zero at the ER.
Protection from excess charges. If a provider bills more than Medicare allows (legal in 42 states), Plan G pays the overage. Plan N leaves it to you.
Set-and-forget budgeting. Once you’ve met the Part B deductible, your out-of-pocket cost is $0 for the rest of the year.
15-25 % cheaper premiums in many ZIP codes. In New York, for example, the average Plan N quote is about $260/month versus $325+ for Plan G.
States like New York and Connecticut ban excess charges, wiping out one of Plan N’s risks.
Healthy retirees who see the doctor only a few times a year may pay less in copays than they save in premium.
Add your annual Plan N premium.
Estimate doctor & ER copays (be honest!).
Add a small buffer in case a provider hits you with excess charges while traveling.
If that total is still below the Plan G premium, Plan N wins. Otherwise, Plan G’s peace of mind is probably worth the extra dollars.
After your six-month Medigap Open-Enrollment window, moving between plans usually means medical underwriting. If your health changes, you might not be able to land Plan G’s richer benefits later—so weigh the long game before defaulting to Plan N.
Bottom line:
Pick Plan G if you want zero surprises, travel often, or live in a state that allows excess charges.
Pick Plan N if premiums are sharply lower in your area, you’re healthy, and you’re comfortable with the small copays.
