Follow our simple guide!

There are three main options when ordering a replacement ID card:

Stay Up to Date on Medicare!

Join the Fair Square Medicare Newsletter to stay informed on cost savings, changes to Medicare, and other valuable healthcare information.
Online: The quickest and most convenient way to order a new Medicare card is by using your personal

myMedicare.gov

account. If you don't have an account, you can create one on the official Medicare website.
Phone: You can also order a new Medicare card by calling the Social Security Administration at 1-800-772-1213 (TTY: 1-800-325-0778) if you're deaf or hard of hearing. Representatives are available Monday through Friday, from 7 AM to 7 PM.
Social Security Office: If you prefer to handle things in person, you can visit your local Social Security office. Use the

Social Security Office Locator

to find your local office.

Verify Your Identity

When you request a new Medicare card, you will likely need to provide some information to verify your identity. This can include your full name, Social Security number, date of birth, and other relevant details. Be prepared to answer questions to verify your identity.

Replacing a Lost or Stolen Card

If your Medicare card was lost, stolen, or damaged, you can order a replacement without changing any of the information on it.

Changing Personal Information

If you need to update personal information, such as your name or address, you must follow a different process. This may require providing documentation to verify the changes.

Guard Your Information

Make sure to keep your Medicare card and Social Security number safe. Only share this information with trusted healthcare providers.

Beware of Scams

Medicare card requests are free of charge. Be cautious if anyone tries to charge you for this service or asks for personal information like your Medicare or Social Security number over the phone or via email.

In Conclusion

Requesting a replacement Medicare ID card is a simple process you can complete online, over the phone, or in person. To be safe, always refer to official government websites or contact authorized representatives for accurate and up-to-date information on ordering a new Medicare card. If in doubt, please give us a call at 1-888-454-2383. Our Medicare Experts are here to help!
